We want to make the provision of Health and Social Care in the Community more accessible, easier to deliver, more efficient, more transparent and more satisfying for all involved.
A holistic approach
We believe it’s possible to achieve this by deeply understanding the needs of Clients, the motivations of our Caregivers and the complex challenges faced by our ICBs. We’re creating processes that address all these needs - using some clever tech to connect it all together.
To do this, we must view the challenges holistically. We are starting to expand the capabilities of the system to tackle other issues within Health and Social Care in the Community.
This includes; the complexity of community nursing operations, Care Home booking delays, assisting Ambulance trusts to reduce their response times and improving delayed transfers of care.
The Greener Care Collaborative study, funded by The Health Foundation, evidenced the possibilities to address the massive inefficiencies in our Healthcare systems. Careguider provides a perfect solution, enabling hidden capacity to be unlocked and Health and Social Care services to be transformed."
